ما هو بروبريك أقراص وشراب وفيم يستخدم؟ 
- يحتوي بروبريك على مادة بامبیوترول هيدروكلوريد وهي مادة تنتمي المجموعة موسعات الشعب الهوائية والتي تعمل عن طريق ارتخاء الشعب الهوائية في الرئتين مما يجعلهما أوسع. - يستعمل مستحضر برودريك لعلاج مشاكل التنفس في المرضى المصابين
بالربو وغيرها من الحالات المشابهة. 
 ما يجب معرفته قبل استخدام بروبريك أقراص وشراب:

 لا تستخدم بروبريك في حالة وجود حساسية لمادة

بامبیوترول هيدروكلوريد أو أي من المكونات الأخرى الغير فعالة.

 عند استخدام مستحضر بروبريك عليك بإخبار الطبيب

  •  إذا كنت تعاني من داء السكري، فقد تحتاج لمزيد من تحاليل نسبة
  • السكر بالدم. 
  •  إذا كان هناك تاريخ مرضى القلب مثل عدم انتظام ضربات القلب أو
  • الذبحة الصدرية.. 
  •  إذا كان هناك زيادة في إفرازات الغدة الدرقية. 
  •  إذا كان هناك مشاكل بالكبد أو الكليتين - إذا كنت ستخضع لتخدير كلي. 

تناول أدوية أخرى مع بروبريك :

قبل استخدام مستحضر بروبريك يجب عليك إخبار الطبيب أو الصيدلي عن كل الأدوية التي تتناولها أو تناولتها مؤخرا ، بما في ذلك تلك الأدوية التي تصرف دون وصفة طبية وخاصة إذا كنت تتناول الأدوية الإستيرويدية (مثل البريدنيزولون) ، الثيوفيللين أو الأمينوفيللين لعلاج مشكلات التنفس ، مدرات البول ، العقاقير المحتوية على مثبطات مستقبلات بيتا.

 تناول بروبريك  اثناء الحمل والرضاعة الطبيعية

- إسألي طبيبك أو الصيدلى للحصول على المشورة قبل أخذ أي دواء 
- يجب عليك إخبار الطبيب قبل استخدام بروبريك إذا كنت حاملا أوتنوين الحمل أو عند حدوث الحمل أثناء تناولك لمستحضر بروبريك 
- لا توجد معلومات إذا ما كانت مادة البامبيوترول يتم إخراجها في لبن الأم، لذا يجب إخبار الطبيب قبل الرضاعة الطبيعية 

معلومات هامة حول بعض مكونات بروبريك أقراص

تحتوي أقراص بروبريك على سكر اللاكتوز، فإذا كان لديك مشاكل في هضم بعض أنواع السكريات فعليك بإخبار الطبيب قبل تناول هذا
المنتج.
تأثير بروبريك على قيادة السيارات أو استخدام الآلات:
لم ترد دراسات توضح تأثير بروبريك على القدرة على قيادة
السيارات أو استخدام الآلات. 
كيفية استخدام بروبريك أقراص وشراب:
يجب استخدام بروبريك أقراص وشراب تماما كما وصفه الطبيب. يجب عليك مراجعة الطبيب أو الصيدلي إذا كنت غير متأكد من طريقة الاستخدام. 

 الجرعة المعتادة هي:

- يفضل تناولها قبل النوم بوقت قصير ما لم يوص الطبيب بغير ذلك. 
- في حالة بروبريك أقراص يتم تناول الأقراص كاملة بدون مضغ مع
كمية كافية من الماء 
- يمكن تقسيم الأقراص إلى نصفين في حالة الضرورة 

الجرعة المعتادة للبالغين والأطفال (۱ - ۱۲ عام): 

- الجرعة المعتادة هي 10 مجم (۱۰ ملليلتر بروبريك شراب أو قرص
بروبريك 10 مجم مرة واحدة يوميا. ويمكن للطبيب زيادة الجرعة إلى ۲۰ مجم (۲۰ ملليلتر بروبريك شراب أو قرص بروبريك ۲۰ مجم) مرة
واحدة يوميا وذلك بعد مرور من أسبوع إلى أسبوعين من بداية العلاج 
- في حالة وجود انخفاض في وظائف الكلي فقد يقرر طبيبك استخدام |
5 مجم (5 ملليلتر برودريك شراب أو نصف قرص من بروبريك 10 مجم مرة واحدة يومية ويمكن للطبيب زيادة الجرعة إلى 10 مجم (۱۰ ملليلتر بروبريك شراب أو قرص بروبريك 10 مجم مرة واحدة يوميا وذلك بعد مرور من أسبوع إلي أسبوعين من بداية العلاج - للأطفال الشرقيين يجب ألا تزيد الجرعة عن ۱۰ مجم (10 ملليلتر
بروبريك شراب أو قرص بروبريك 10 مجم مرة واحدة يوميا. 

 الجرعة المعتادة للأطفال (۲ - 5 سنوات):

- ۱۰ مجم (۱۰ ملليلتر برودريك شراب) مرة واحدة يومية - للأطفال الشرقيين يجب ألا تزيد الجرعة عن 5 مجم (5 ملليلتر بروبريك
شراب) مرة واحدة يوميا. 

 في حالة استخدام جرعة زائدة من مستحضر بروبريك:

- في حالة تناول جرعة زائدة عن طريق الخطأ فقد يزيد هذا من الآثار الجانبية الأكثر شيوعا ومن هذه العلامات: الغثيان ، العصبية ، الصداع سرعة ضربات القلب ، تشنج عضلات اليدين والقدمين والساقين ، دوخة
ونعاس غير عادي - عند ظهور أي من هذه الأعراض بعد تناول جرعة مفرطة من مستحضر
بروبريك يجب إبلاغ الطبيب فورا 

 يجب عليك الاتصال بالطبيب فورا في حالة:

- إذا ما ازدادت الحالة سوء أ. 
- إذا لاحظت تکرارية استيقاظك في الليل مع صعوبة في التنفس أو أزيز
في الصدر
- إذا لاحظت وجود ضيق بالصدر 
- إذا لاحظت أن مستحضر بروبريك لم يحسن من عملية التنفس. 
- إذا لاحظت أي من الأعراض السابقة فإن هذا يدل على أن الربو لم يتم
السيطرة عليه ولذا فقد تحتاج إلى علاج آخر أو علاج إضافي
إذا كان لديك أي أسئلة أخرى عن استخدام هذا المنتج، اسأل طبيبك أو الصيدلي. 

في حالة نسيان تناول مستحضر بروبريك في الوقت

المحدد عليك بتناول الجرعة المنسية بمجرد أن تتذكر إلا إذا اقترب ميعاد الجرعة التالية فعليك أن تنتظر حتى يأتي موعدها ولا تتناول جرعة
مضاعفة لتعويض الجرعة المنسية 

التوقف عن استخدام مستحضر بروبريك أقراص وشراب:

لا تتوقف عن استعمال مستحضر بروبريك أقراص أو شراب إلا بعد
التحدث مع الطبيب 
 الآثار الجانبية المحتملة: -
 مثل جميع الأدوية يمكن أن يتسبب مستحضر بروبريك في أعراض
جانبية قد تختلف من مريض إلى آخر وليس من الضروري أن تصيب
جميع المرضى
- عادة ما تختفي مثل تلك الأعراض الجانبية في غضون أسبوع إلى
أسبوعين من العلاج - يراعي إخبار الطبيب إذا لاحظت أن أي من الأعراض الجانبية المذكورة استمر معك أو ازداد سوءا أو لاحظت أي أعراض جانبية غير تلك
المذكورة في هذة النشرة 

تفاعلات حساسية:

- قد تحدث تفاعلات حساسية مع بعض المستحضرات مثل بروبريك أقراص أو شراب وليس من المعروف تحديد سبب ذلك ، فإذا لاحظت حدوث تفاعلات حساسية مرتبطة باستخدامك لمستحضر بروبريك فتوقف عن الاستعمال واتصل بالطبيب فورا - قد تشمل تفاعلات الحساسية الآتي: انتفاخ في الوجه - طفح جلدي - مشاكل في التنفس - انخفاض في
ضغط الدم (شعور بالدوخة أو إغماء). - قد يحدث ضيق في الشعب الهوائية غير معروف السبب. فإذا لاحظت
حدوث ذلك بمجرد تناولك لجرعة بروبريك، توقف عن استعمال
المستحضر وأخبر الطبيب فورا 

 يجب عليك اخبار الطبيب مباشرة إذا لاحظت: 

- وهن في العضلات. - شعور بالعطش أو الوخز الأمر الذي قد يعني نقص مستوى البوتاسيوم
في الدم. ليس من المعروف سبب حدوث هذه الحالة ولكنها تحدث في حالة الربو الشديد أو تناول المستحضرات الإستيرويدية أو الثيوفيللين أو الأمينوفيللين أو الأقراص المدرة للبول. 
- مشاكل في القلب مثل عدم انتظام ضربات القلب. 

 أعراض جانبية أخري محتملة الحدوث: 

  • شائعة جدا (أكثر من 1 من كل 10 أفراد): قد تحدث رعشة أو صداع
  • أو الإحساس بعدم الراحة. 
  • شائعة: (أقل منا من كل 10 أفراد): خفقان أو صعوبة في النوم
  • أو تقلص عضلي. 
  •  غیرشائعة: (أقل من 1 من كل ۱۰۰ فرد): سرعة ضربات القلب أوعدم انتظام ضربات القلب أو شعور بالاضطراب 
  •  أعراض جانبية أخرى قد تحدث ولكن بمعدل غير معروف 
  1. - غثيان أو فرط في النشاط. 
  2. - ألم في الصدر (بسبب مشاكل في القلب) - عليك بإخبار الطبيب إذا لاحظت أي من الأعراض السابقة ولكن لا تتوقف عن تناول العلاج إلا بعد الرجوع إلى الطبيب.

۵- كيفية تخزين بروبريك أقراص وشراب

لمستحضر بروبريك أقراص (۱۰ و ۳۰ مجم)؛ 

- علية كرتون تحتوي على شريط ( PVC/ Alu ) به ۱۰ أقراص ونشرة داخلية 
- يحفظ في درجة حرارة لا تتجاوز ۳۰م، في مكان جاف 

: المستحضر برودريك شراب

 - علبة كرتون تحتوي على زجاجة بها ۱۲۰ ملليلتر شراب ونشرة داخلية. 
- بحفظ في درجة حرارة لا تتجاوز ۳۰م 
- لا تستخدم بروبريك أقراص وشراب بعد تاريخ إنتهاء الصلاحية المبين على العبوة 
- لا يستخدم إلا تحت إشراف طبي.

 لمستحضر بروبريك أقراص (۱۰ و ۲۰ مجم): يحتوي كل قرض على: المواد الفعالة 
- لمستحضر برودريك 10 مجم أقراص بامبیوترول

Composition: Tablets Each tablet contains: Active ingredient: - For Probric 10 mg Tablets: Bambuterol hydrochloride.

5 mg Inactive ingredients: Disodium edetate, Glycerol, Sorbitol, Sodium benzoate, Citric acid monohydrate, Cherry flavour, Ponceau 4R, and Purified water. Pharmacological action: - Pharmacotherapeutic group: selective B2-agonists, bambuterol. - Bambuterol is an active precursor of the selective B2-adrenergic agonist terbutaline. Bambuterol
is the bis-dimethylcarbamate of terbutaline, and is present in the formulation as a 1:1 racemate. - Pharmacodynamic studies have shown that after oral administration of bambuterol to guinea pigs, a sustained protective effect was achieved against histamine-induced bronchoconstriction. The bronchoprotective effects seen after oral administration of bambuterol is related to the generation of Terbutaline, as were the secondary effects (effects on other organs). - Pharmacodynamic studies have been conducted in asthmatics and healthy volunteers. The effects observed were bronchodniation, tremor and increases in heart rate. The metabolic effects included a small increase in blood glucose, while the effect on serum potassium was negligible. In short-term studies on lipoprotein metabolism, an increase in HDL cholesterol has been observed. In conclusion, all pharmacodynamic effects observed can be ascribed to the active metabolite terbutaline. Pharmacokinetics: - On average, 17.5% of an oral dose is absorbed. Approximately 70 - 90% of the absorption occurs
in the first 24 hours. - Bambuterol is metabolised in the liver and terbutaline is formed by both hydrolysis and oxidation. After absorption from the gut, about 2/3 of Terbutaline is first-pass metabolised, bambuterol escapes this first-pass metabolism. Of the absorbed amount, about 65% reaches the circulation. Bambuterol therefore has a bioavailability of about 10%. - Protein binding of Bambuterol is low, 40 - 50% at therapeutic concentrations. - The terminal half-life of bambuterol after an oral dose is 9 - 17 hours. - Studies on the effects on plasma cholinesterase showed that bambuterol inhibited activity, but
that this was reversible. - All categories of subjects studied were able to form terbutaline in a predictive way except for
liver cirrhotics. Preclinical safety data: - Bambuterol has not revealed any adverse effects which pose a risk to man at therapeutic dosages
in the toxicity studies. - There is no evidence from the preclinical safety data to indicate that bambuterol cannot be used
in man for the intended indications with sufficient safety. Indications: Management of asthma, bronchospasm and/or reversible airways obstruction Dosage and administration: - Probric is formulated as a tablet and should be taken once daily, shortly before bedtime. The dose
should be individualised. - Adults: The recommended starting doses are 10 mg - 20 mg. The 10 mg dose may be increased
to 20 mg if necessary after 1 - 2 weeks, depending on the dinical effect. - Children 6-12 years: The recommended startina dose is 10 mg (10 mi). Doses may be increased
to 20 mg if necessary after 1 - 2 weeks, depending on the clinical effect. Because of differences in kinetics, doses above 10 mg (10 ml) are not recommended in oriental children.


- Children 2-5 years: The recommended starting dose is 10 mg (10 ml), 5 mg (5 ml) in oriental
children because of differences in kinetics. In patients who have previously well tolerated Bz-agonists, the recommended starting dose, as well as maintenance dose, is 20 mg. -Elderly: Dose adjustment is not required in the elderly. - Significant hepatic dysfunction: Not recommended because of unpredictable conversion to
Terbutaline. - Moderate to severely impaired renal function (GFR< 50 ml/min): It is recommended that the
starting dose of Probric should be halved in these patients, Overdosage: Overdosing may result in high levels of terbutaline. - Symptoms:
The signs and symptoms described here have been recorded after terbutaline overdose. Possible signs and symptoms: Headache, anxiety, tremor, nausea, cramps, palpitations, tachycardia, cardiac arrhythmias. A fall in blood pressure sometimes occurs. Laboratory findings: Hypokalaemia, hyperglycaemia and lactic acidosis sometimes occur.
Overdose of Probric is also likely to cause a prolonged inhibition of plasma cholinesterase. - Management: Mild and moderate cases: Reduce the dose, Severe cases: Administration of activated charcoal if ingestion is recent. Determination of acidbase balance, blood sugar and electrolytes. Monitoring of heart rate and rhythm and blood pressure. Metabolic changes should be corrected. A cardioselective B-blocker (e.g. metoprolol) is recommended for the treatment of haemodynamically significant cardiac arrhythmias. The B-blocker should be used with care because of the possibility of inducing bronchoconstriction. Serum potassium levels should be monitored. If the B2 mediated vasodilation contributes
significantly to the fall in blood pressure, a volume expander should be given Contra-indications: Probric is contraindicated in patients with a history of hypersensitivity to any of their ingredients Side Effects: - Adverse events are listed below by system organ class and frequency. Frequencies are defined as: very common (21/10), common (21/100 to < 1/10), uncommon (21/1000 to < 1/100), rare ( 1/10,000 to < 1/1000), very rare (< 1/10,000) and unknown (cannot be estimated from available data). - Most of the adverse reactions are characteristic of sympathomimetic amines. The intensity of the
adverse reactions is dose-dependent. Tolerance to the effects has usually developed within 1-2 weeks.

Muscle cramps and bone disorders Drug-Drug Interactions: - Bambuterol may interact with suxamethonium (succinylcholine). A prolongation of the musclerelaxing effect of suxamethonium of up to 2-fold has been observed in some patients after taking Bambuterol 20 mg on the evening prior to surgery. The interaction is dose-dependent. It is due to the fact that plasma cholinesterase, which inactivates suxamethonium, is partly, but fully reversibly, inhibited by bambuterol. In extreme situations, the interaction may result in a prolonged apnoea time which may be of clinical importance. - Bambuterol may also interact with other muscle relaxants metabolised by plasma cholinesterase. - Beta-receptor blocking agents including eye drops, especially non-selective ones, may partly or totally inhibit the effect of beta-stimulants. Therefore, Probric and non-selective B-blockers should not normally be administered concurrently. Probric should be used with caution in patients receiving other sympathomimetics. - Hypokalemia may result from B2-agonist therapy and may be potentiated by concomitant treatment with xanthine derivatives, corticosteroids and diuretics.


Precautions and warnings: - As terbutaline is excreted mainly via the kidneys, the dose of Probric should be halved in patients
with moderately to severely impaired renal function (GFR s 50 mL/min). -Care should be taken with patients suffering from thyrotoxicosis. - Cardiovascular effects may be seen with sympathomimetic drugs, including bambuterol.
There is some evidence from post-marketing data and published literature of rare occurrences of myocardial ischaemia associated with beta agonists. Patients with underlying severe heart disease (e.g ischaemic heart disease, arrhythmia or severe heart failure) who are receiving Probric should be warned to seek medical advice if they experience chest pain or other symptoms of worsening heart disease. Attention should be paid to assessment of symptoms such as dyspnoea
and chest pain, as they may be of either respiratory or cardiac origin. - Due to the hyperglycaemic effects of B2-stimulants, additional blood glucose measurements are
recommended initially when Probric therapy is commenced in diabetic patients. - Due to the positive inotropic effects of B2-agonists these drugs should not be used in patients
with hypertrophic cardiomyopathy. - B2-agonists may be arrhythmogenic and this must be considered in the treatment of the
individual patient. - Unpredictable inter-individual variation in the metabolism of bambuterol to Terbutaline has been shown in subjects with liver cirrhosis. The use of an alternative B2-agonist is recommended
in patients with cirrhosis and other forms of severely impaired liver function. - Potentially serious hypokalaemia may result from B2-agonist therapy mainly from parenteral
or nebulised administration. Particular caution is advised in acute severe asthma as this effect may be augmented by hypoxia. The hypokalaemic effect may be potentiated by concomitant treatment with xanthine derivatives, corticosteroids and/or diuretics. It is recommended that serum potassium levels are monitored in such situations. - If a previously effective dosage regimen no longer gives the same symptomatic relief, the patient should urgently seek further medical advice. Consideration should be given to the requirements for additional therapy (including increased dosages of anti-inflammatory medication). Severe exacerbations of asthma should be treated as an emergency in the usual manner. - Patients with rare hereditary problems of galactose intolerance, the Lapp lactase deficiency or
glucose galactose malabsorption should not take this medicine. Pregnancy and lactation: - Unless there are compelling reasons, avoid in pregnancy, lactation and women of childbearing
potential who are not taking adequate contraceptive precautions. Although no teratogenic effects have been observed in animals after administration of bambuterol, there is no experience of use in human pregnancy. Terbutaline, the active metabolite of bambuterol, has been in widespread clinical use for many years and may be considered in such patients. Terbutaline should be used with caution in the first trimester of pregnancy. Maternal B2-agonist treatment may result in
transient hypoglycaemia in pre-term newborn infants. - It is not known whether bambuterol or intermediary metabolites pass into breast milk. Terbutaline
does pass into breast milk, but an effect on the infant is unlikely at therapeutic doses. Effects on ability to drive and use machines: None stated. Patient information: Patients are instructed to consult their physician if they experience severe, troublesome adverse reactions, become pregnant, intend to become pregnant, or intend to breastfeed while taking Probric, or they have any other questions.
